For NDIS clients we have a preference that a service agreement is developed. A service agreement is a non-binding mutual agreement between us and you. Our service agreements cover; the supports we have agreed to provide, their cost, who will provide them and where, your responsibilities and our responsibilities.
As part of developing a service agreement, we collect information such as your NDIS goals and other goals, relevant information about your health condition, and your support network. We also collect your NDIS number, plan dates and your contact details. For information about how we manage your private information, please refer to our Privacy Policy.
For capacity building sessions (therapy sessions), the therapist will likely develop a support plan in collaboration with you. A support plan documents your goals, level of importance and how the goal will be reached, amongst other things.
